New patients

All new patient appointments will require a referral from a treating practitioner i.e General Practitioner (GP), Specialist.

You may email your referral to info@vivwong.com.au.

Once received, your referral will be reviewed and we will contact you thereafter to organise an appointment.

Meanwhile, kindly complete the Patient Registration and Australian Pelvic Floor Questionnaire.

If you prefer to fill in these forms in our rooms, please arrive at our rooms 10 minutes prior to your appointment.

Referrals

A referral is required in order for you to receive the Medicare rebate that is eligible to you. If your referral is invalid on your day of your appointment or if a new referral is not provided, you will be required to submit your claims to Medicare on your own. A receipt will be provided to assist with your claim.

What to bring on your first visit:

  • Referral letter from your GP/specialist (if this has not been previously forwarded to our rooms)
  • Any relevant medical reports/letters or results
  • Medicare card
  • Private Health Fund card (if available)
  • Veteran Affairs Card
  • Photo ID i.e drivers licence

Billing and payment options

Payment is to be made in full on completion of the consultation. We accept EFTPOS, Visa and Mastercard.

We are able to process your eligible Medicare claims directly from our rooms.

Prescription scripts

If you need a repeat script, please visit your local GP. If you have an appointment scheduled, Dr Wong will be able to provide you with a script at that time.

Cancellation Policy

When you book your appointment, you are holding a space in our calendar that is no longer available to our other patients. In order to be respectful of your fellow patients, please call our practice at 1300 848 966 as soon as you know you will not be able to make your appointment.

If you must cancel your appointment, we respectfully request 48 hours’ (2 business days) notice. In the event that we do not hear from you by that time, your appointment will be cancelled and a cancellation fee will apply.

For booked procedures, please ensure that you give us 5 business days’ notice if you would like to reschedule.
